PCs are unable to enter the Black Mist and exit the underworld

Description
When a player dies they are transported to an Underworld location. To exit the underworld the character must locate the Black Mist. Entering the mist will teleport the player out of the Underworld.
The bug here is that entering the mist does nothing. No visible error occurs, no messages are displayed, and nothing happens.
